Staff Biography

Chase Gray, RN

Program Manager, PCMH Foundations

Chase manages a caseload of medical practices involved in one or more of HealthTeamWorks’ improvement activity programs.  She coaches clinical and support personnel in these practices as they work to become patient-centered medical practice homes. Using an evidence-based approach to quality improvement (QI), Gray facilitates change from cultural, operational and technological perspectives. As program manager, she also trains and supervises other HealthTeamWorks coaches in their work with client practices.

Chase began her professional career in Washington, D.C., doing international development work for a USAID contractor, American International Health Alliance. She was responsible for planning, implementing and monitoring a variety of health programs throughout the former Soviet Union and Eastern Europe that focused on transitioning healthcare systems. This experience inspired her to become a registered nurse.

Returning home to Colorado in 2007, Chase worked as a RN in a variety of healthcare settings, including adolescent mental health, clinical research, developmental disabilities and as a quality-improvement coordinator for a home health agency.

Chase earned a dual bachelor's degree in international affairs and Russian language and culture from the University of Colorado in 1999 and completed her BSN at Johns Hopkins University in 2007. She served as a United States Peace Corps health volunteer in Uzbekistan from 2000-2001.
