Kari Loken

PCMH Project Manager 

Kari supports the Patient-Centered Medical Home (PCMH) project for HealthTeamWorks. She serves as the project manager for the Systems Transformation team, facilitates the Patient Advisory Committee and offers patient experience training to practices. Kari is passionate about providing premier, patient-centered care through customer service and by empowering the patient to be an active member of their personal health care team. Prior to joining HealthTeamWorks in 2007, she had 10 years experience as a program manager and customer service trainer and supervisor.
