Nicole Deaner, MSW

Residency Manager, PCMH 

Nicole Deaner joined HealthTeamWorks in 2009 and is currently the Residency PCMH Project Program Manager.  In this role, she continues to coach in the family medicine residency practices in Colorado as well as doing some consulting with residency practices outside of Colorado.  Nicole very much enjoys working in partnership with the University of Colorado Denver, Department of Family Medicine to help promote Patient Centered Medical Home principles in residency practices, most of whom serve a vulnerable patient population.   Nicole is passionate about quality improvement and finds it rewarding to help practices improve their work with patients by becoming more team based and patient centered in their approach.

Before coming to HealthTeamWorks, Nicole served as Executive Director of an elder care company, HouseWorks, in the Boston area.  She also worked with vulnerable populations through her work with the New England Chapter of the Multiple Sclerosis Society, in the Denver adolescent residential treatment community working with vulnerable children and her advocacy work with at-risk and women involved in the Denver criminal justice system.  

Nicole received her B.S. from Illinois State University in 1994 and later earned her M.S.W. from the University of Denver in 2001.
