The Colorado Trust has been a stalwart partner since 2004. Its initial funding allowed HealthTeamWorks to establish our infrastructure, develop a strategic plan and facilitate the development and dissemination of evidence based guidelines. More recently, the Trust's support enabled HealthTeamWorks to convene one of the early Multi-State, Multi-Stakeholder Patient-Centered Medical Home Pilot projects and provide medical practices the coaching and technical support to help ensure that the program is a success. 

HealthTeamWorks would also like to express gratitude for the very generous support of the Colorado Health Foundation. Its funding has been invaluable for our general operating needs. It has also ensured our ability to foster access to quality, coordinated healthcare in specific regions of the state. The Colorado Health Foundation's support has enabled us to develop programs and provide quality-improvement coaching to primary care practices and communities to evolve into patient-centered medical homes and integrated medical neighborhoods.

We thank two other early and consistent funders: the Colorado Department of Public Health and Environment (CDPHE) and the Caring for Colorado Foundation. CDPHE has made financial contributions over many years for clinical guideline development, dissemination and implementation at the practice level.
