Population health starts with individual patients. Sharing goals and trends using personal health indicators can activate patients to improve their health. The graphs show the type of report generated monthly for each practice working with HealthTeamWorks.

Demonstrating Measurable Impact at the Practice Level

Each practice collects, reviews and submits quality measures monthly to identify and correct system deficits that impede improvement. The coach reviews the measures and assists the practice with work-flow redesign to correct problems as they arise. This quality reporting is also invaluable when a practice applies for NCQA recognition. 

Demonstrating Measurable Impact at the Community or Aggregate Level

Using the registry capability of our client practices, HealthTeamWorks collects and reports clinical quality measures at the patient and practice levels. We also report data in aggregated forms that helps identify problems and aids program decision-making. We can examine reports by region, by coach, by program and other parameters to improve healthcare quality at every level. This is a hallmark of the accountable care organization.
