Healthcare reform has lent momentum to the Patient-Centered Medical Home (PCMH) movement. Although many primary care practices understand the need for change, most lack the resources or infrastructure to implement the modifications required to establish a PCMH.
HealthTeamWorks is ideally positioned to assist primary care practices in their evolution to the PCMH model, thanks to our experience with Improving Performance in Practice (IPIP), a national quality initiative in which we convened the participants and managed the program's implementation in Colorado. We brought the skills and experience gained from IPIP into our PCMH Foundations program for the redesign of primary care practices. The Foundations program incorporates the chronic care model and applies tools, systems and support mechanisms to help providers deliver consistent, high-quality care to all patients, all the time.
Like the IPIP program, the PCMH Foundations Program provides in-office assistance to practices, focusing on efficiency, technology, clinical quality and satisfaction for both patients and the healthcare team. Thanks to grant-subsidized funding, primary care practices in Colorado receive coaching and technical assistance free of charge.
Practices initially develop efficient, patient-centered systems around diabetes, smoking cessation and asthma. They apply the strategies to other conditions moving forward. The PCMH Foundations Program aims to dramatically improve patient outcomes by:
- Transforming healthcare delivery
- Integrating quality improvement and data collection methods into practices
- Incorporating population-based strategies for patient management
- Developing and applying strategies to expand and sustain improvements to care
- Increasing efficiency and satisfaction for both patients and the healthcare team
The PCMH Foundations Program implementation includes:
- In-practice quality-improvement coach
- Disease registry functionality and data reporting (required)
- Practice team redesign
- Consultation on work-flow analysis, chronic care model, open-access scheduling, practice culture change, EHR conversion and/or optimization
- Focus on system and work-flow redesign
