The National Committee for Quality Assurance (NCQA) designed its recognition process for the patient-centered medical home (PCMH) to help practices define the standards for PCMH structure and articulate the necessary change processes. HealthTeamWorks understands that many practices and organizations are eager to evolve into PCMHs and document the transformation. Process changes must take place for work flow and increased efficiency, but culture change also is pivotal. It includes:
- The move away from a physician-oriented focus to a patient-centric focus that involves the patient with the care team;
- Patients' involvement with their health and wellness, and acknowledgement of a long-term relationship with their primary care providers; and
- The primary care provider as the first resource that patients turn to for almost any healthcare need.
Adoption of this model requires visionary leadership, communication with and education of the staff, management of the practice's entire patient population and, most importantly, the ability to manage change as a team. HealthTeamWorks' experienced quality improvement Coaches can help with the transformation of any practice, regardless of its starting point. We can also assist you with the lengthy and complicated application process for NCQA recognition (please note there is a separate fee for this service).
NCQA sets three levels for recognition of the PCMH. Each level requires the completion of a number of steps.
Learning Series for Practice Transformation and Achievement of NCQA Recognition
HealthTeamWorks is uniquely qualified to offer a specially designed learning series for practices and organizations interested in pursuing NCQA recognition. In some markets, this program starts with a full day introductory conference to build the groundwork for the application and PCMH practice transformation. The conference sets the stage for participant's successful practice submission for medical home recognition, but more importantly, it creates the foundational understanding of what it takes to transform to the model of patient-centered care. We encourage practices to bring key members of their leadership and clinical teams so they can champion the effort within the practice. In those markets where we are unable to do a kick-off program, participants may be interested in the webinar series.
Webinar Series
The NCQA webinar series consists of five one-hour webinars delivered over a ten-to-twelve week period of time. This webinar series provides in-depth descriptions of the requirements for NCQA recognition as well as how the recognition process provides the practice opportunity for developing systems structures and processes to promote transformation to the patient centered model of care. Each presentation will be followed by a two to three week time period for the practice to begin developing their response for the standards that were discussed in the previous webinar.
Between each of the webinar series there will be a one hour “open line” phone consultation available to the participant group. We will provide a conference phone line with a password for participants to call in at the designated time.
As an additional benefit and for a separate fee, HealthTeamWorks will make consultation time available from a “practice coach” to review your practice application and all supporting documentation prior to submission to NCQA. To allow for completion of the review and have time to make any necessary changes or suggestions for your application we must be notified as to interest and receive the paperwork at least 2 weeks prior to your anticipated submission date.
