In this foundational program, participants will learn processes for identifying patients appropriate for care management services, distinguish between episodic and longitudinal care management, develop workflows and approaches, and learn how to engage with patients to develop personalized care plans. This program contains 15 courses each presenting detailed information with a mix of activities and checks for understanding. To support the application of the learning, participants will also receive access to HealthTeamWorks’ Solutions Center learning community.
