Effective community-clinical partnerships to address social determinants of health can powerfully impact an organization’s ability to manage the total cost of care. Even when practices are successful in improving quality of care, adapting to changing payment models, and addressing burnout, many remain challenged to meet the needs of their highest risk patients.
