Once population health needs are understood, the next step is to engage with the community and other local stakeholders to progress ideas for redesigning services. Insight from our PCH community of practice shows 25% of all initiatives are focused on redesigning existing models. The top five are focussed on mental health, care of older people, complex long term conditions, wellness and diabetes and MSK. Our support includes:

• Facilitating engagement sessions for co-production with service users and carers

 • Running working groups bringing colleagues together to develop future care models

• Reviewing existing models to identify gaps and areas for improvement

• Sharing insight from PCH new models of care

• Cost and spend analysis of current service provision

Beyond Primary Care Round Table 2019

There is a real appetite from the acute, community and mental health trust leaders in London to proactively and productively engage with the development of Primary Care Networks. There are barriers to making this a reality including a lack of bandwidth in Primary Care, the number of PCNs, Trusts are engaging with and historic lack of relationships. However, the group identified that there were some positive steps that could be taken to work together by focusing on population health and that they would be keen to do this in collaboration with NAPC and the Leadership Academy.
