HealthTeamWorks hosted the first quarterly in-person Learning Network meetings for 3 cohorts of practices in the program in March of 2018.   

The Transforming Clinical Practice Initiative (TPCi) is a national initiative funded by The Centers for Medicare & Medicare Services (CMS) to prepare clinicians to be successful in new value-based compensation models by providing onsite facilitation and coaching and clinical health information technology support. As a leader of practice transformation, HealthTeamWorks provides practice facilitation support to three Learning Networks of primary care and specialty practices located in Colorado, including:

  • 9 Mental Health Centers
  • 10 Boulder Valley Independent Physician Association (BVIPA) practices
  • 17 Primary care and specialty practices

In the first in a series of learning network events last month, participants responded that they were highly satisfied with their experience with HealthTeamWorks seasoned Practice Facilitators and Clinical Health Information Technology Advisors (CHITAs). In our evaluation, participants reported the benefits of the events to include:

  • The hands-on learning experience
  • Networking with specialist partners in their learning networks
  • The facilitation of quality improvement tools to develop a vision for transformation and create PDSAs to achieve the vision
  • Evidence-based best practices to enhance access to care
  • Strategies to reduce the overall healthcare cost and utilization for patients

With significant increases in knowledge and confidence in their transformation work, the practices are eager for their next learning network event and continued practice transformation support provided by HealthTeamWorks.
