The HealthTeamWorks® approach matches and paces the organization’s priorities to optimize both clinical outcomes and financial performance. With more than 20 years of experience in patient-centered practice and system transformation, we apply proven best-practices to our network client engagements. We deploy our decades of experience in proven, evidence-based practice to map out – and support – operational plans and activities for success and business excellence.
HealthTeamWorks® works with our practice and network clients where they are on the journey from volume-to-value and empower delivery system changes, provider collaboration and market leading improvements to achieve the 5-Part Aim of increased clinical quality, lowered costs, improved patient experience, workplace satisfaction, and thriving healthcare businesses.
We drive measurable improvements in healthcare delivery through deep client collaboration and engagement. HealthTeamWorks partners with practices, physician groups and integrated delivery networks to provide the services, tools and training to achieve the 5-Part Aim in value-based payment models.
While direct attribution of specific implementation to outcomes requires robust evaluation methodologies, these highlights reflect key improvements achieved by our clients. Appropriate measures and joint expectations are defined in initial project or model design.

HealthTeamWorks® was launched in 1996 as the Colorado Clinical Guidelines Collaborative (CCGC). We are still well-known for our guidelines work today, and we work with two trusted partners to update and maintain our numerous clinical guidelines:
