Elizabeth Kraft

Dr. Elizabeth (Cissy) Kraft is a Board-Certified Family Medicine Physician with a track record of proven success in utilization management, data-driven cost of care programs and leading value-based interventions targeting at-risk patient populations. She is a strategic thought leader and practical tactician with team orientation to execute on identified concerns.

Dr Kraft was one of the co-founders of and currently serves as the Board Chair of HealthTeamWorks. Dr. Kraft is an experienced NCQA surveyor in Health Plan, Population Health, Medicaid and Exchange Accreditation and Patient Centered Medical Home (PCMH) Recognition. She is currently a Medical Director with CHN (Colorado Health Neighborhoods) performing Population Health transformation in clinical practices in the Centura Health system. She was with Anthem Blue Cross for over twelve years in the commercial managed care field. She was a key contributor for the Colorado Multi-Payer PCMH Pilot; for which program outcomes were published in Health Affairs September 2012. Dr. Kraft served on the Board of the Colorado Prevention Alliance which won the prestigious Root Cause Coalition Inaugural award in 2018.

Dr. Kraft merged high technology with advanced care management in the successful implementation of “In Home Hospitalization”, an innovation in health care delivery. Other first in nation innovations include creation of a High Needs High Cost member and provider incentive program for a commercial population, implementation of a medically tailored meals program in collaboration with a nonprofit community provider and hospital system in a commercial population, and was in a leadership role for an award winning community collaboration to advance preventive services in an environment of public/private partnerships.
