HealthTeamWorks Supports Practices in Delaware SIM

This year, The Medical Society of Delaware (MSD)/MedNet has contracted with the State of Delaware, Department of Health and Social Services, Delaware Health Care Commission (HCC) to support primary care practice transformation in Delaware as part of the Delaware State Innovation Model (SIM). Primary care practice transformation is a key component of Delaware’s Health Innovation Plan, adding to the State’s goal of improving healthcare quality and experience, while reducing costs for all Delawareans.

HealthTeamWorks^® is partnering with MSD/MedNet to support Delaware physicians as they strive to enhance the health of Delaware’s communities. On September 29, the quality improvement teams of 16 practice are pictured here during their most recent learning collaborative. We celebrate their accomplishments and recognize their hard work as they continue transforming their practices, thereby helping Delaware to become a healthier state.
