Effective care management is a key driver to quality care, lower costs, and increased patient and provider satisfaction. At HealthTeamWorks® we've learned a dedicated resource for care management allows for targeted interventions to accelerate cost and quality objectives. In this training, Care Managers will acquire skills for integrating into the care team, establishing and implementing care management processes, tools, and resources, and educating and activating patients to adequately address their health care needs.


A 2-day In-Person Workshop Focused On

  • Defining the role of the Care Manager in the primary care setting and outline strategies for integrating into the existing care team
  • Gaining a deeper understanding of core population health principles (e.g. empanelment & risk stratification) and how this applies to managing the care of your patients
  • Developing efficient processes and workflows to ensure provision of safe, high-quality, and comprehensive care
  • Developing an approach for engaging with patients to develop self-care plans and provide appropriate self-management support

Essentials of Care Management

A 15-course learning path focused on the items covered in the 2-day workshop plus:

  • Identifying and assessing patients who would benefit from Episodic and/or Longitudinal Care Management
  • Developing strategies for collaborating with the expanded care team including the patient and caregiver
  • Establishing and documenting processes for both Episodic and Longitudinal Care Management
  • Establishing the patient relationship and utilizing evidence-based techniques to engage in collaborative care planning
