Effective care management is a key driver to quality care, lower costs, and increased patient and provider satisfaction. At HealthTeamWorks® we've learned a dedicated resource for care management allows for targeted interventions to accelerate cost and quality objectives. In this training, Care Managers will acquire skills for integrating into the care team, establishing and implementing care management processes, tools, and resources, and educating and activating patients to adequately address their health care needs.

Attendees Will

Define the role of the Care Manager in the primary care setting and outline strategies for integrating into the existing care team

Gain a deeper understanding of core population health principles (e.g. empanelment & risk stratification) and how this applies to managing the care of your patients

Develop efficient processes and workflows to ensure provision of safe, high-quality, and comprehensive care


Develop an approach for engaging with patients to develop self-care plans and provide appropriate self-management support
